Diabetes Audit:

The number of people with diabetes in Ireland is estimated to be more than 200,000, with the majority of patients between the ages of 20 and 79. Due to increased levels of poor diet and obesity, the prevalence of diabetes is expected to rise by 30% by 2020. The Audit tool which allows a better management of diabetes patients. Atrial Fibrillation Audit:

Atrial fibrillation is the most common cardiac arrhythmia in general practice. Atrial fibrillation is a major modifiable risk factor for stroke. A simple sample audit on AFib is available on www.icgp.ie/audit. Safe Prescribing in Pregnancy Audit:

Decisions relating to the use of medications can be challenging while prescribing for the women in pregnancy. The previous research in Ireland revealed that excluding folic acid, 46.8% of pregnant women were prescribed at least one medication. To identify pregnant women who may be taking medication that could be potentially harmful, an audit tool for safe prescribing in pregnancy is available. This tool uploads anonymised data to our iPCRN server which then generates and emails back a detailed practice report which may then be used for audit. This tool will analyse anonymised data from patients who are currently recorded as receiving maternity care at your practice. No patient identifiable information is included in this upload.

Heart Failure Audit:

Cardiovascular Risk Audit:

GPs can do an audit on their patients at risk of cardiovascular disease (ICPC code K22 - those with a risk of cardiovascular disease either because they have any risk factor, have existing cardiovascular illness or a family history). Dementia Audit:

Ireland is predicted to have the largest growth in the older population of all European countries in the coming decades. Dementia is one of the most common and serious disorders in later life. As part of the PREPARED project in UCC, an audit has been devised for practices – it is available on www.icgp.ie/audit. GPs can use the audit tool on dementia available under their IPCRN section of their software to receive a comparative report from their practice saving time on the data extraction/collection aspect and gives you time to focus on quality improvements. This tool uploads anonymised data to our iPCRN server which then generates and emails back a detailed practice report which may then be used for audit. PHEMI Audit:

People with enduring mental illness have higher morbidity and mortality from chronic diseases than the general population, and this results in a significantly reduced life expectancy. The vast majority of the gap in life expectancy is accounted for by physical illness. An IPCRN audit tool in now available to permit you to run an audit on the physical health of your patients with enduring mental illness, via the Physical Health Monitoring tab. Ireland has the fourth highest prevalence of asthma world-wide, affecting an estimated 450,000 people. Poor asthma control has significant health, social and economic costs. MECC Audit:

Lifestyle factors are linked to many chronic diseases in our population. In order to assist you to record lifestyle factors in your practice management system, we have engaged with the software vendors and commissioned Socrates, Helix Practice Manager and Health One to create Physical Health Monitoring Tab in their systems which collates all relevant data in one place and also permits you to record brief interventions and referrals to community and voluntary agencies. An audit tool is available which provides you with a comparative report on lifestyle factor recording among your practice population linked to relevant guidelines. Please note that only patients coded with the appropriate condition will be included in the report provided by IPCRN. If the number of patients is less than you anticipated in your practice, you should check if you have patients coded with this condition – in some instances an IPCRN ‘Finder’ tool is available on your system that when run will prompt you with patients who may have the condition to help you identify same and code them.
